Case study: Chronic GERD with four-year PPI dependence in a 45-year-old

A classical homeopathy case study. All identifying details — name, exact age, specific city — have been removed or lightly changed. The clinical pattern, remedy picture and arc are faithful to the actual case.

Presentation

A 45-year-old senior sales manager presented with a four-year history of daily pantoprazole (40 mg) for GERD. Three previous taper attempts over two years had each ended in rebound symptoms within two weeks. Endoscopy twelve months prior had shown mild oesophagitis without Barrett's change, no H. pylori. Gastroenterology had suggested either indefinite PPI or an open-fundoplication discussion. The patient wanted to try classical homeopathy before the surgical consultation.

Case-taking — what the first consultation opened up

The patient was hurried, driven, irritable under pressure, impatient at home, slept poorly (woke 3 am most nights, could not return to sleep), drank 6 to 8 cups of strong coffee a day, had a late dinner habit (often after 10 pm), and alcohol on weekends. Symptoms were classical nocturnal GERD — worse within an hour of lying down, worse after late meals, worse in bouts of over-work. Mental picture: zero tolerance for inefficiency in others, blunt in confrontation, held the family financially, under-slept but refused to admit it. Craving: spicy food and stimulants.

Remedy rationale

The constitutional picture — hurried, irritable, over-stimulated, over-worked, coffee-dependent, waking 3 to 4 am, worse from lifestyle indiscretion — was a textbook Nux Vomica picture. Pantoprazole continued; the taper plan was explicit and gastroenterology-led, to begin only once the pattern had visibly settled.

Potency and repetition are deliberately not stated here. Classical homeopathy matches remedy, potency and repetition to each case; a remedy name without that matching is a reference, not a prescription.

Follow-up arc

At six weeks the patient reported markedly steadier days; the 3 am waking had moved to 5 am and sleep was better overall. Coffee had reduced from 8 to 3 cups a day on his own initiative, not instructed. Gastroenterology agreed to begin a stepwise PPI taper (40 mg to 20 mg to intermittent to as-needed) over the following six months. At four months the patient was on 20 mg every other day; at six months he was on as-needed basis only. At nine months he had not used pantoprazole in four weeks; mild acidity on heavy or late meals was managed with lifestyle alone.

Outcome

Twelve months after the first consultation, the patient was off daily PPI for six continuous months, used an antacid occasionally after very heavy meals or late dinners, sleep had steadied, coffee was down to 2 cups a day, and the fundoplication discussion had been deferred. Gastroenterology continued annual oversight.

What did not happen — the honest frame

The underlying over-worked lifestyle pattern remained a struggle — the patient still worked long hours, still ate late more often than ideal, still faced recurrence risk if lifestyle slid. The honest framing — "the remedy supports the pattern, the lifestyle has to meet it halfway" — was reiterated at every follow-up.

Discussion and learning points

A Nux Vomica GERD case where the constitutional remedy and a genuine (if partial) lifestyle shift combined to achieve a PPI taper that had failed three times before. The case also illustrates that classical homeopathy for lifestyle-driven chronic patterns works best when the patient is willing to meet the remedy with some lifestyle work — not perfect compliance, but a direction of travel.

Compliance notes

  • Patient identifying details (name, exact age, specific city) anonymised.
  • No prescription detail (potency, dosing schedule) stated — classical homeopathy prescribes individually.
  • Allopathic treatment described was continued throughout; dose changes stayed with the treating specialist.
  • Outcomes described are one patient\'s experience, not a promise about yours — classical homeopathy is individualised, so no two cases run the same arc.
