Case study: Chronic migraine with visual aura and menstrual trigger in a 28-year-old

A classical homeopathy case study. All identifying details — name, exact age, specific city — have been removed or lightly changed. The clinical pattern, remedy picture and arc are faithful to the actual case.

Presentation

A 28-year-old marketing manager presented with a seven-year history of right-sided throbbing migraine with visual aura (zigzag lines preceding the headache by 20 minutes), nausea, photophobia, and menstrual trigger. Attacks had been four to six per month over the past year, each lasting 24 to 48 hours. Sumatriptan rescued acute attacks; a preventive beta-blocker trial had been discontinued after six months of modest benefit. MRI brain was normal. Neurology continued to oversee the case.

Case-taking — what the first consultation opened up

The patient was reserved, perfectionist, driven, with a persistent low-grade anxiety that was not situational. Grief history: her father's death four years prior, which she had not processed openly. Salt craving was marked; thirst for cold water was intense; worse in sun; worse from grief; migraines preferentially right-sided. Pre-cycle week brought additional migraines. Mental picture: closed emotionally, averse to pity, cried alone, held weight well in sport, slept lightly. The visual aura always preceded the headache; the headache was throbbing "like hammers," worse from any noise.

Remedy rationale

The combination — right-sided migraine with visual aura, worse pre-cycle, worse in sun, salt craving, grief history, closed emotional register — pointed to Natrum Muriaticum as the matching constitutional remedy. Sumatriptan continued as rescue; the plan was to reduce frequency over 3 to 6 months, not to abort attacks pharmacologically from the homeopathic side.

Potency and repetition are deliberately not stated here. Classical homeopathy matches remedy, potency and repetition to each case; a remedy name without that matching is a reference, not a prescription.

Follow-up arc

The first month showed no visible change; two attacks occurred at the usual severity. The second month brought three attacks of noticeably reduced duration (18 hours rather than 36). By the third month attacks were down to two per month, both milder, and the patient had used sumatriptan twice rather than four times. By the sixth month attacks were running at one or two per month, pre-cycle only; the general low-grade anxiety had also softened — a secondary gain that the case-taking had anticipated. At nine months the patient had gone three consecutive weeks without an attack, the longest stretch in seven years.

Outcome

Twelve months into consistent constitutional homeopathy, migraine frequency had settled at an average of one every six weeks, almost always pre-cycle only, duration shortened, sumatriptan usage reduced from 4 to 6 doses a month to 1 to 2 doses a month. The patient continued neurology follow-up annually.

What did not happen — the honest frame

The pre-cycle trigger did not fully resolve — menstrual migraines remained the main residual pattern. The patient and the homoeopath discussed whether a cycle-targeted remedy change (Lachesis or Cimicifuga constitutional picture) might be worth considering, deferred for the moment while the Natrum Muriaticum pattern was still improving. Sumatriptan remained on hand; the goal was never to eliminate it.

Discussion and learning points

A right-sided, grief-anchored, Natrum Muriaticum migraine case — one of the most classical presentations in the homeopathic migraine repertory. The lesson for both patient and practitioner was that the shift was gradual (3 to 6 months) and secondary (anxiety also softened, salt craving slightly reduced), not confined to the migraine count alone.

Compliance notes

  • Patient identifying details (name, exact age, specific city) anonymised.
  • No prescription detail (potency, dosing schedule) stated — classical homeopathy prescribes individually.
  • Allopathic treatment described was continued throughout; dose changes stayed with the treating specialist.
  • Outcomes described are one patient\'s experience, not a promise about yours — classical homeopathy is individualised, so no two cases run the same arc.
